Can some one please help me code this note::confused:

Preop and Postop Diagnosis:

Operation: Excision of draining sinus tract

There was a small sinus tract at the inferior aspect of old sternotomy incision. It was excised. A pocket of approximately 20 mL of purulent material drained and culture taken. Wound were packed with Oodoform gauze.
