have any instruction regarding add site modifier (RT/LT/T-modifier or F-modifier) used to HCPCS codes (L3260)?

Hi. L3260 is not covered by Medicare. Only a few payers cover it. This is normally patient responsibility with or without signing an ABN.

Could you included the billing scenario?
