Wiki Skilled Nursing Facility and NP billing

Has anyone information on how to bill the services of a Nurse Practitioner who visits patients and the physician is also at the site and does have face-to-face with the patient. I'm also interested - I know we can't bill "Shared/split" but have any of you billed "Incident to" at the Nursing Home or does every visit performed by the NP have to be billed under NP. I'd appreciate any billing, documentation guidance .......
