I had a patient that was seen in the nursing home that slept through the entire visit. The nurses suggested that the patient not be awaken. There was no physical exam, but the provider did stop medications. I am wondering if this is even something I can charge for? Patient does have dementia, so would the nurse qualify as an independent historian?
