A gentleman who is currently a skilled nursing home resident recently decided to change his primary care physician to our doctor. It is not a new SNF admission although the doctor has never seen this patient. Would the first visit be an initial nursing facility visit or a subsequent?
 
Subsequent Visit; because the level of service reported by the physician should include the services related to the ongoing care the resident is receiving in the Nursing Faciltiy. The patient is not new to the Nursing care plan in the facility.
An Initial Nursing Facility care service can be performed for a New or Established patient on re-admission to the Nursing care.
