Does anyone know how you would bill for a visit where the physician was out of the building, but saw the patient via skype or IPAD?

99444 states non face to face physician encounter, on line visit, which is e-mail and the last line states or other similar electronic communications network. I think this fits.

Thanks for this. It looks like this is for an estbl patient; do you know of any for a new patient/consultation?

From the description of the code and the notes for on-line evaluation in the CPT book, you can use 99444 for either new or established patients. The one word of caution is that if the online evaluation refers to another E/M service performed by the same MD within the past 7 days, then this service is not separately reportable and is covered by the previous E/M code.
