Is anyone having trouble getting paid for smoking cessation G0436 or 99406 with an E/M for Medicare? They pay on the smoking cessation counseling and do not pay on the E/M stating it is not payable when done with another service. Do I need to put the 25 modifier with the E/M?

Yes we need to append 25 modifier for E/M, to explain that it is significantly and separately identifiable service.

smoking cessation

I have tried it with mod 25 and the denial CO182 stated modifier was invalid on the date of service. I also am struggling with the obesity counseling G0447. I included the body mass index, and still got denied. Can anyone help?
