When coding smoking cessation, does the time documented in the notes need to be a specific time, or is a range of time (5-6 minutes) ok to use? Is anything else required for documentation of smoking cessation?

Time should be in the med record documentation for the day. As example doc could write "I discussed/counseled the patient stop smoking for 5 minutes, 10 minutes Etc". then can bill with CPT codes. Also use the definitive dx code of F17 dx. block or Z72 dx codes per notes in the record.
