Can you code G0436/99407 alone without and E/M CPT code when a patient comes in for an office visit with commercial/Medicare insurance?

If they come in and only discuss smoking without doing any other E/M exam, diagnosis, or preventive care; then yes, you can (and should) code for the smoking discussion only. Be sure the report notes how much time is spent in the discussion as that is what the procedure code is based on for those.
