For an admit to SNF from a hospital can the admitting physician reference a history and physical done by the hospital admitting physician? They have a statement saying they have reviewed and agree with the findings of the admit to the hospital. My understanding is they would need to document their own.

There are several assessments that have to be done for an admit to a nursing homein addition to the H&P. So no you are correct, they have to do their own.
