SNF Visit-please help

I have a physician who sees patients in the nursing facility for routine foot care. Can a subsequent visit (99307) be billed or can this only be billed if there is a separately identifiable problem?
 
if the patient is medicare then 99307 is not covered and the SNF would have to pay the physician, otherwise if the there is a procedure i will just will that. That would we handle ours.
