Social History: patient lives in XX city

Can that alone count as social history? :confused:

IMO, I would not count that - now, there are areas of the city that may affect the health of the patient, and if this connection is documented, then yes, I would use it. But in your example of just ______ City - no, I would not.
 
Thanks, Lance. That settles it for me then. There's a bit of an argument going on about it with my coworkers. I agree with you - it's not enough without further documentation.
