Hi,
Please clarify if this statement can be considered for Social History- ' Unknown if ever smoked"
 
I would count it as social history. Smoking status is generally considered social history, and the fact that this is unclear, doesn't negate the credit (in my opinion). Lack of information is still information! Good question, though.
