I am trying to figure out the difference between stream of thought and contnent of thought, with in the single system Psychiatric Exam bullets. Are they under the same bullet? is there a difference, so I can count an additional bullet?

Stream of thought and "train of thought" are similar. It refers to how patients process current information as it comes to them, and relates the interconnection between the information. Thought content would elaborate as to whether this train of thought was logical or coherent. They're both considered thought processes within the '97 psychiatric exam, so I'd probably give only one bullet, unless the provider described hallucinations, phobias, etc., which falls under "Description of abnormal or psychotic thoughts".
