I have been denied payment for CPT 92019 25 as included with 67314 rt
I'm I billing correctly? should I use modifier 57 or 59

You must use -59. Modifier 25 is for procedures that have a 10 day global period. Modifer -59 is the best choice in this situation.
 
99219 is obs care. If it is a post op for the strabismus, you need to code 99024. If the 99219 was prior to the surgery, same day, I'd do 57m, decision for surgery. I use 59 for procedures hitting against each other, not for an E/M hitting against something.
