We preform internal chart audits. We are a multiple physician ENT practice. I'm wondering about EMR templates. What is the general consensus about provider templates? Should providers pre-populate exam templates with normal findings and update them abnormal findings? Or should a provider have to document each and every element of the exam in the EMR?
