I answered this under your other question as well, but no you do not have to be a CPC first to take a specialty exam.

I would suggest strong familiarity with coding, because specialty exams are tough. (I have not taken the COPC, but I have taken the CHONC and CGSC.) But you don't have to take the CPC first.
