I have a physician that is doing a physical on patients with several chronic but stable issues. She spends a significant amount of time discussing diet, medication management, answering patient questions, etc. She therefore is charging an additional office visit to account for this additional time. If she properly documents her time spent, would this be appropriate?

Is this an annual? if so then no you can bill the annual only unless there is an office visit for a specific abnormality discovered or patient symptomatic complaint. The purpose of an annual is to check all exisiting issues and go over any questions and treatment plan.
