I have a scenario and I'm curious as to what others think about it. A patient is discharged and the discharge summary is a split/shared note that does not meet the requirements for the physician. The physician also has a progress note on the same DOS that is not a split/shared note and says that should count as him having the substantive portion for the split/shared discharge. My thoughts are, if a progress note cannot be used for a discharge, then it shouldn't count as his substantive portion. There is nothing in the progress note about discharging the patient on that DOS. What are your thoughts on this?
