Sports and cdl physicals - charting in the EMR

Can you use the sports and cdl forms as charting? My provider says it's double charting in filling out the forms and charting in the EMR.
 
Sports physical

Have the forms scanned as a document into your EHR system, then attach as applicable to individual charts. An alternative is to scan completed forms as an attachment to patient charts like you would driver's licenses.
