I have a superbill in front of me and all it says on it is "Spinal Cord Leads X2, flouroscopy, programming" and it says in front of all of these "trial only". This is for a Medicare PT. Do I code 63650 with C1787, C1778, C1897 and a 76000? or is the flouroscopy included? Can I code C1778 and C1897 together or does it have to be one or the other? How do I code for 2 leads. Do I put 2 units for 63650 or do 63650 and a 63650-51?? Please help!
