Is there a standard way to submit recurring visits to Medicare? (or will they accept what ever the hospital sends) The example I have is for External Beam Radiotherapy (EBRT) where the patient can have multiple lesions treated on multiple days. CPTs used are 77402, 77407 and 77412. Any help is greatly appreciated.
 
Have you checked for any NCDs/LCDs about it? If I remember correctly, a while back I came across possibly an LCD (for my area) regarding EBRT billing... not sure though. Maybe a better approach would be to research billing EBRT in general and you'd probably find the answer as to how to submit your charges.
