Patient comes in and the nurse does a pregnancy test to confirm pregnancy then starts the global package and schedules appointment for following week with provider for exam. Can we bill for the pregnancy test and if so can we bill it under the provider that will follow the patient throughout the global or do we have to bill it under the provider that was on call that day the test was performed?

Has the patient been evaluated by a provider in your office at all? NP or MD? You cannot bill services provided by a nurse if the patient has never been examined by the physician/NP.
