Hello all,

I'm having a bit of trouble with the billing of these codes (61796-61799). They say 61796 & 61798 cannot be billed "more than once per course of treatment." (or more than 4 times for 61797 & 61799)

What exactly defines the course of treatment? Are these notes to say that you shouldn't bill these codes more than once per day, or is more than once per as long as the doctor is operating the specified lesions?
