I have a patient who had a partial sternotomy with subsequent conversion to formal sternotomy for emergent/salvage repair of spontaneous acending aortic tear with institution of femoral atrial cardiopulmonary bypass, who died on the table. I'm really at a loss, not only for the codes but for the modifiers. Can anyone help me?
 
Ascending Aorta repair

Not sure if you still need some help but look at procedure code 33860. This may fit but it all depends on documentation and without seeing the report can't give you the exact code but this may point you in the right direction.

No modifier should be needed unless the procedure was not completed, then maybe (53?) then again it all depends on the documention and what all the physician did to repair the dissection/tear.
