Wiki Stress echo dx problem

I am having a problem with finding a dx for the 93018 and 93016 portion of my stress echo. The reason for the tests are usually chest pain, palpitations, tachycardia, or bradycardia. None of which are on the LCD for these codes. Why are these codes not acceptable dxs?
