I code pro fees and have only been capturing cpt code 93018 for the interpretation and report of the stress test in an OP setting. My question is how do I know if my physician was also the supervising physician at the facility and available in order to code 93016 for the supervision portion of pro fees? Does the interpretation report need to state he was the supervising physician? Is there a separate report? Is there other documentation? Not sure I know what I am looking for to capture 93016. Thank you
