Dx:
1. submacular hemorrhage, left eye
2. vitreous hemorrhage left eye
3. exudative age-related macular degeneration left eye

Operative:
1. pars plana vitrectomy
2. submacular tissue plasminogen activator injection
3. sulfur hexafluoride gas

Procedure in detail:
a speculum was placed. standard pars plana vitrectomy trocars were set up. upon inspection, there was vitreous hemorrhage. there was a large area of submacular hemorrhage. pars plana vitrectomy was performed. PVD was present, and vitrectomy was performed. Using a submacular cannula, submacular tPA was injected. after appropriate injection, vitrectomy was performed, and air-fluid exchanged was performed. the instruments were removed, ad 20% SF6 gas was exchanged for air.

assigning CPT 67036 is correct? Thanks in advance.
