I coded a 99232-57 & a major surgery CPT code, as of 7/1/13 Medicare is denying the 99232 stating the modifier is incorrect for that CPT code. Medicare customer service told a biller that audit finding as of 7/1/13 no longer allow a mod 57 with a subsequent hospital care code but won't assist in which modifier is needed. I can't find any news on this change on there web site, has anyone came across this problem?
