Patient being followed by XYZ specialty for hospital days 1-4 all by Dr A. On hospital day 5, Dr. B of XYZ specialty sees the patient. Is this considered 'new problem to examiner' or would this just be established problem?

I do this everyday, not sure why today I am second guessing myself but any input is appreciated.

I personally would rather it be new to examiner but I can say that our Medicare carrier WPS states that all providers in the same group/same specialty are seen as the same individual so this would be an established problem.
