I would appreciate any help in coding the below procedure.

Indications for the procedure.
56 y.o. female with The primary encounter diagnosis was Carotid-cavernous fistula.

PROCEDURE:

1.- Right superior orbital vein direct approach, phlebotomy, for access for endovascular procedure.
2.- Intraoperative doppler guidance to find vein.
3.- Right orbital doppler-ultrasound guided cut-down phlebotomy, to access the superior ophthalmic veins for vascular access into the orbit and cavernous sinus.

PROCEDURE IN DETAILS:

Incision planned to be over right eyelid, middle half, guided by ultrasound doppler.

All soft tissues brought down with monopolar, using Colorado Micro tip, in low setting, respected levator palpebral, and cut thru orbicularis. Ultrasound microdoppler used for guidance intra op. Found vein, but very tortuous and small, so pursued it thru the periorbita, and found better segment. 2 loose loops of silk left around, not fastened.
