Our medical director is out of town until August and we are trying to figure out how to bill commercial plans for our Midlevels. I think we should bill it under the physician that is in the office on that day the patients is seen with the SA modifier?

Does anyone have any opinions on this?
 
If incident-to requirements are met then yes bill using the NPI of the provider that is in the office at the time of the encounter. Otherwise use the NPI ofbthe midlevel
