Wiki Supplies Included with Anesthesia

Hello-
I'm trying to determine what supplies are included with anesthesia and what would be separately billable. The CPT guidelines indicate that supplies and materials provided by the physician OVER AND ABOVE those usually included with the office visit or other services rendered may be listed separately. Can anybody tell me or direct me to a resource that would tell me what's usually included?

Are you referring to supplies in a pain practice or supplies in the Operating Room......not sure what you are asking?

Thank you for your clarification - I'm actually looking for a listing that would encompass any procedure that would include billing for anesthesia - need to know what supplies are separately billable for all circumstances. Would there be a resource that would tell me this?
