There's a primary provider and an assistant for the surgery along w/the surgeon attending. What is a surgeon attending mean? Do I need to bill surgeon attending or is there a modifier for the surgeon attending?

Are there two surgeons or three surgeons involved?

You may have a surgeon plus assist. Or you may have a co-surgery. Or you may have a team.

Please clarify.
