One of our physicians did an incision and drainage of a scrotal abcess on 10/4, on 10/6 took the pt back to the OR and did debridement of the abscess and placement of wound vacuum,
and on 10/10 took the pt yet again to the OR this time for removal of the wound vacuum and closure of the wound

My question is what of this can be billed for the 10/6 & 10/10 services or is it all considered part of the original 10/4 service ?

Thanks for any ideas/thoughts on this would be much appreciated

I think you can bill for the debridement (11004-11006) with a 78 modifier. I am not sure about the wound vacuum. I would think the removal of the wound vacuum would be included in the insertion of the vacuum.
