Op note states...RT hydrocelectomy, excision of a 1.5cm x 1.5cm condylomatous lesion located at the left base of the penile shaft which was excised sharply and excision site aggressively cautherized then a small scrotal lesion was excised using Bovie cautery

Thoughts on billing this ??
