Wiki Surgery&Radiology: Regarding Contrast material and Injection

Kindly clarify the following please:

1) INJECTION: "Injections are usually included in the radiology codes".
Does this mean, if no reference in the code discription, then, do we take it for granted that it is included?

2) CONTRAST MATERIALS: Can we code separately if the code description does not refer to inclusion of the contrast material?
Are there exceptions to this rule? ( other than oral and rectal contrast materials which are always a part of the procedure)

This I am asking for, on the CPC examination point of view. I need the correct info sothat I do not go down in the exam!!!
