I am in need of assistance of coding out a harvesting tendon transfer of plantaris tendon, left leg.
Here is op note:
The decision was made to reinforce the tendon with biologic grafting as well as supplementation and reinforcement the plantaris tendon. The plantaris tendon was found to be healthy in appearance and strong enough for supplementation. The plantaris tendon was harvested and then placed in sterile saline for later use.
I would use CPT 20924 for the above.
Now this is were I am a bit confused:
Next the plantaris tendon was woven through the Achilles and wrapped around in Burrito style for supplementation and reinforcement and strengthening purposes.
Next, a couple of Amniofix grafts were also wrapped around the tendon for enhanced healing purposes and decreased scar tissue build up. Also, a Graftjacket was used for supplementation of the repair.
It should be noted that postoperatively, the patient did develop a small hematoma about the incision from the gastroc recession. This area of skin was found to be very thin and friable and therefore another Amniofix graft was placed subdermally to enhance healing and decreasing the scar tissue build up in this area.......
The above has me baffled as I received a response from another site and graftjackets I guess are to be precerted by hospital for billing of material; however, we should be able to bill for the application????
My thought would be to code 17999, unlisted procedure for the amniofix grafts and for the graftjacket. I am only billing the application as the supply was provided by hospital not our physician.
Any input would be greatly appreciated!
