Wiki Suture charge for ischemic ulcer

An 80 yo came into the Wound Care Center regarding an ischemic ulcer at his ankle. This was a longstanding problem. The doctor decided to loosely suture the wound, and tighten the sutures on a weekly basis. Has anyone had to code a scenario like this? Is there a CPT code for tightening sutures? From the research that I've done, I advised them to charge for the suturing as they normally do, and the tightening of the sutures would be included in the E/M code. Anyone with advice regarding this?
