This could mean a number of things: "equal to or greater than," "radiates to," "thus" or "it follows that."
 
An arrow -> is often used to show a relationship between two statements. It appears that is how your doc is using this symbol. He is indicating the statement "continue same meds, stable" is connected to the dx of "hypertension."

In order to avoid confusion and/or potentially dangerous outcomes, many practices and facilities have written standards of documentation including acceptable abbreviations and symbols. If your practice doesn't have these, you may want to suggest it. It saves a lot of headache and could potentially save lives.
