Hi,

I need a coding help, Patient coming with syncope, done MRI brain during hospitalization and finding out Stroke. Physician doubts the syncope is due to stroke.The patient is having hypertension and the physician again saying may the syncope is due to this hypertension. Final documentation says Syncope due to Hypertension versus Stroke. My doubt is regarding the assignment of Principal diagnosis. Can we assign the sycope as PDx, based on the ICD Guideline Symptom following by contrasting diagnosis. "When a symptom(s) is followed by contrasting/comparative diagnoses, the symptom code is sequenced first. All the contrasting/comparative diagnoses should be coded as additional diagnoses"
