hello sir, Medicaid-north Carolina state , till July 2023 they paid and after July they are not paying , the reason they giving " The procedure/revenue code is inconsistent with the type of bill" invalid type of bill".CO-16 means that the payer needs more information.
Medicaid varies by state - you may get more specific responses if you share what state's Medicaid plan you're billing.

1500 Form we billed …This isn't a modifier issue - the denial is telling you that there's an invalid type of bill.
Do you bill on a UB04? What type of bill are you using, and what revenue code is being billed?
