Hello,

Can someone explain to me what the difference in coding would be for a personal history of non drug induced anaphylaxis Z87.892 and a anaphylactic reaction due to unspecified food, sequela T78.00XS?

NP came in and has a history of an allergic reaction to a melon (lips and tongue swell and shortness of breath) that happened a year ago. Her new problem is that she touched a melon and broke out in hives a day ago. I am not sure what code to use in this instance.

Anyone? Thoughts?
 
The T78.04- code is for the anaphylaxis and the S would be for any late effects of the anaphylaxis. The Z code is for the history code of having the reaction. If the patient arrives with hives the provider would need to relate the hives to the melon as an allergen. The hives are not sequela of the anaphylaxis. So this would be the T code for the food allergy first followed by the rash second. As long as documentation supports
