Can anyone tell me when to use I25.810 (Atherosclerosis of coronary artery bypass graft(s) without angina versus T82.857A (Stenosis of Cardiac Prosthetic devices, implants and grafts, initial encounter)?
The Op note documentation for the Heart Cath simply reads "graft to the right coronary is occluded"
 
T82.857- would be used for an in-stent stenosis (restenosis) of an artery other than a coronary or vascular artery. The initial stenosis would be the coded with the I25.8--. Hope this helps!
