When using the table of risk. Do you take into account the whole note with multiple conditions or just the part that pertains to the office visit.
 
That catagory is labeled Presenting Problem(s), so if it is related to or a condition effecting the presenting problem, or if it is examined/treated in addition to the presenting problem I give credit for it. But if it is just part of the list of Active Problems and has no bearing on the visit I don't include it.
