Wiki TAVR converted to SAVR; how to bill the professional component?

If a TAVR procedure (requires an IC & CTS) is converted to an open procedure; how does the IC bill? Does the IC bill for the TAVR procedure with modifier 53? And would the CTS bill the SAVR code only with modifier 22?
