Is it possible to bill TCM 99495 and 99350 on the same day without a modifier? Thank you for your help!

No, you cannot bill both of these together with or without a modifier. If your encounter meets the TCM then bill for the TCM because the description in the TCM states "face to face" so your home visit is your face to face. Also, your POS will establish that this is a home visit. So i would not bill for 2 E/Ms on the same day. I would bill for the most accurate per your documentation.
