Is TCM billable if the patient goes to SNF in Nursing Home?

what about if the go into the Nursing Home but a NON SNF unit?
 
TCMs are billable after discharge from an IP stay. Since Nursing Home and SNF stays are also considered inpatient, and the discharge must be to a home setting (home, assisted living, domiciliary home) you cannot bill TCM when the patient is discharged from the hospital into a SNF.
