When billing out a 99495 or 99496 do you count the discharge date as day 1? Also, if a patient was discharged but has a follow up with their PCP the day after they were discharged, does the phone call/contact with someone have to be done separately? Or can that appointment, meeting with a nurse before-hand at check in count at that communication? Thanks for your help!
