Wiki TCM visit and EKG???

Patient has their face to face Transition Care visit with the provider, and the provider does an EKG because of patients current DX. Do I bill a 99496 AND a 93000? Does the 93000 get a modifier? Is the EKG inclusive to the TCM visit?
ANY help is most appreciated.
 
The date of service for the TCM would not be the date of the face to face. So, you would bill the EKG on the date it was performed and the TCM would be billed with date of service 30 days post discharge.
